The long Covid problem might be bigger than we thought.

A large study has revealed that one in three Covid-19 survivors have suffered symptoms three to six months after getting infected, with breathing problems, abdominal symptoms such as abdominal pain, change of bowel habit and diarrhoea, fatigue, pain, anxiety and depression among the most common issues reported.

Researchers at the University of Oxford, the National Institute for Health Research and the Oxford Health Biomedical Research Centre studied symptoms in more than 270,000 people recovering from Covid-19 and found that the nine features of long Covid were detected by clinicians more frequently in those who had been hospitalized, and slightly more often in women.

But Dr. Max Taquet, National Institute for Health Research academic clinical fellow and one of the authors of the study, said the results show long Covid affects a significant proportion of people of all ages. “We need appropriately configured services to deal with the current and future clinical need,” he said.

The study did not explain what causes long Covid symptoms, how severe they are or how long they will last, but it did show that people recovering from Covid were more likely to suffer long-term symptoms than those who had the flu.

Dr. Amitava Banerjee, a professor of clinical data science at University College London who was not involved in the study, said this finding is “yet another arrow in the quiver against bogus ‘this is just like flu’ claims.”

The symptoms people experienced varied, and many patients experienced more than one. Older people and men were more likely to have breathing difficulties and cognitive problems, whereas young people and women reported more headaches, abdominal symptoms, anxiety and depression.

The authors stressed that although the number of such incidents was higher among the elderly and those with more severe initial illness, people who had suffered a mild disease, children and young adults also experienced long Covid.
The accompanying data showed that as many as 46% of children and young adults between the ages of 10 and 22 had experienced at least one symptom in the six months after recovering.

This risk of long Covid highlights why is it so important to protect children and young people from the coronavirus, even though the study said most don’t suffer from severe illness.

Cases among children have been soaring in the US since the more contagious Delta coronavirus variant became the country’s dominant strain in July. The American Academy of Pediatrics reported 206,864 weekly cases among children on Monday. That was a slight decline compared to the previous week, but a 188% increase since the week of July 22.

The data comes as Pfizer/BioNTech said Tuesday that they had begun submitting vaccine data on children aged 5 to 11 to the US Food and Drug Administration for review, and expect to submit a request for emergency use authorization in the coming weeks.
The next key question is whether parents will want to get their kids vaccinated. Parents of 5-to-11-year-olds are split on the issue, with 44% saying they are likely to do so and 42% saying they are unlikely to, according to poll results from Axios-Ipsos published Tuesday.

Study data published in i-Perception suggest that the COVID-19 pandemic has altered perceptions of mask-wearing individuals. In a research study conducted in Japan, mask wearing was found to be associated with higher perceived facial attractiveness during the pandemic period compared with the prepandemic period. Mask wearing was also less often associated with “unhealthiness” compared with the prepandemic period and was often associated with good health among survey respondents.

This survey was conducted as an addendum to a 2016 study that assessed the perceived attractiveness of individuals wearing sanitary masks. Per the 2016 study, mask wearing was associated with a decrease in externally rated facial attractiveness. The hypothesized contributing factors for this phenomenon were 2-fold: (1) the effect of occlusion, in which facial features are obscured by the mask; and (2) “unhealthiness priming,” or the perception that an individual wearing a sanitary mask is unhealthy. To test whether these factors persisted during the COVID-19 period, investigators administered 3 online surveys to adults in Japan. In the first survey, participants were asked to rate on a 7-point Likert-type scale the perceived level of attractiveness and healthiness of women wearing black or white sanitary masks. Participants were not shown images. In the second survey, undergraduate and graduate students were shown images of women with and without sanitary face masks and asked to rate attractiveness on a scale of 1 to 100. In the third survey, participants were asked to rate the perceived healthiness of women with and without masks, also on a 100-point scale. The first survey was conducted between June and December 2020, the second between May and July 2020, and the third on January 15, 2021. A chi-square test and residual analysis were performed to compare data from the prepandemic study with results from the current surveys.

A total of 286 adults responded to the first survey, among whom 153 were men and 133 were women. Mean age was 20.05 ± 2.27 years. The demographic features of the cohort were similar to those of the 2016 study. The second survey enrolled 59 students with a mean age of 19.98 ± 1.61 years, among whom 29 were men and 30 were women. Compared with data from the prepandemic period, mask wearing had less of an impact on perceived attractiveness during the pandemic. The effect of mask color was minimal. In the third survey, which enrolled 44 individuals with a mean age of 41.18±9.98 years, mask-worn faces were less likely to be perceived as “unhealthy” during the pandemic period compared with the prepandemic period (P =.002).

Taken together, results from these studies suggest that the perception of mask wearing has been altered by the COVID-19 pandemic. Compared with the prepandemic period, mask-wearing faces were less likely to be perceived as unhealthy or less attractive during the pandemic. Study limitations include the relatively small size of the study cohorts and changes in social norms surrounding mask wearing, which may have prompted individuals to artificially rate mask wearers as more attractive or healthy. Further study may better elucidate the impact of COVID-19 on facial perception.

“[T]his study demonstrated a contextual modulation of facial perception that took place over a short period of time,” investigators wrote. “Long-term measurements of beliefs and perceptions regarding mask-worn faces would provide more information regarding the impact of the epidemic on societies worldwide.”

New findings challenge the belief that half of children with ADHD outgrow the condition by adulthood. Although patients may show intermittent periods of symptom remission, 90% of children with ADHD followed into young adulthood continued to experience residual symptoms, according to the results of a prospective longitudinal study published in the American Journal of Psychiatry.

“It’s important for people diagnosed with ADHD to understand that it’s normal to have times in your life where things may be more unmanageable and other times when things feel more under control,” said lead researcher Margaret H Sibley, PhD, associate professor of psychiatry and behavioral sciences at the University of Washington School of Medicine and a researcher at Seattle Children’s Research Institute.

Previous longitudinal studies on ADHD focused on a single time point and, thus, were not designed to identify patients who experience symptom remission and later develop symptom recurrence, the study authors explained.

To better understand the disease course, researchers examined longitudinal patterns of remission from ADHD among 558 children enrolled in the Multimodal Treatment Study of ADHD (MTA). The children underwent 8 assessments over a 14-year follow-up period starting 2 years after baseline (mean age, 10.44 years) up to 16 years after baseline (mean age, 25.12 years). Participants with fully remitted, partially remitted, and persistent ADHD at each time point were identified using parent, teacher, and self-reports of ADHD symptoms and impairment, treatment utilization, and comorbidity.

Fluctuating Persistence and Remission of ADHD Symptoms Found

Approximately one-third of children with ADHD (31.4%) experienced full remission at 1 time point during the follow-up period, with a range of 1.4% at the 2-year assessment to 18.5% at the 10-year assessment; however, full or partial recurrence of ADHD symptoms was found in 60% of these patients after an initial period of full remission. A majority of patients (63.8%) experienced a fluctuating persistence and remission of ADHD in the absence of recovery. Recovery from ADHD, defined as sustained remission to study endpoint, was found in 9.1% of patients.

The results suggest that “full remission at a single time point should not be conflated with recovery from ADHD,” the authors noted. They called for continued periodic screening for recurrent symptoms and impairments after successful treatment of ADHD.

The findings are limited by the observational study design and discontinuation of follow-up at approximately 25 years of age. The sample included patients with combined type ADHD; thus, the findings may not be generalizable to other ADHD subtypes or presentations. Missing data may also limit the findings.

Opioid tapering may be associated with an increased risk for overdose and mental health crises in patients prescribed long-term, higher dose opioid therapy, according to research results published in JAMA.

Using deidentified medical and pharmacy claims data from 2007 to 2019, researchers conducted a retrospective cohort study to evaluate the potential associations between opioid tapering and rates of overdose and mental health crisis in patients prescribed long-term and higher-dose opioids.

Primary study outcomes include emergency or hospital encounters for drug overdose or withdrawal and mental health crisis, including depression, anxiety, or attempted suicide, during the 12-month follow-up period.

The total cohort included 113,618 patients who had been prescribed a stable, long-term, higher-dose of opioid therapy for at least 12 months; a total of 203,920 baseline periods were contributed (mean per patient, 1.8; median per patient, 1.0). Within this cohort, 54.3% of patients underwent opioid dose tapering were women (vs 53.2% of those who did not).

In total, 18.2% of baseline periods were followed by tapering for 37,170 tapering events; 20.5% were discontinued at some point during the follow-up period. Median maximum velocity of dose reduction was 22.7% (interquartile range [IQR], 15%-41.3%) per month for tapered periods and 3.2% (IQR, 1.7% to-5.9%) per month without tapering.

Researchers found that those who underwent tapering had significantly higher baseline opioid doses. These patients were also more likely to be coprescribed benzodiazepines and had significantly higher baseline rates of overdose, drug use disorder, depression, and anxiety.

During periods after dose tapering investigators identified an adjusted incidence rate of 9.3 overdose events per 100 person-years, vs 5.5 events per 100 person-years in nontapered periods (adjusted incident rate difference [aIRD], 3.8 per 100 person-years; 95% CI, 3.0-4.6; adjusted incidence rate ratio [aIRR], 1.68; 95% CI, 1.53-1.85).

Opioid tapering was associated with an adjusted incidence rate of 7.6 mental health crises per 100 person-years, vs 3.3 events in nontapered periods (aIRD, 4.3 per 100 person-years; aIRR, 2.28 per 100 person-years; 95% CI, 3.2-5.3 and 1.96-2.65, respectively).

In patients who underwent opioid tapering and had 1 or more outcome event during follow-up, median time to first event was 6 months for both outcomes. Results of secondary analyses of individual components for the mental health crisis outcome showed that tapering was associated with depression, anxiety, and suicide attempts (aIRR, 2.46, 1.79, and 3.30, respectively).

Results of analyses of interaction terms between tapering status and other key covariates indicated informative and statistically significant interactions between baseline dose category and tapering status. For both of these outcomes, patients who were prescribed a higher baseline dose had greater risks associated with tapering compared with those prescribed lower baseline doses.

Further analyses of maximum monthly dose reduction velocity in 109,599 patients followed up over 139,941 person-years showed that an incremental increase in maximum monthly dose reduction velocity of 10% was associated with an increased aIRR for both overdose and mental health crisis (aIRR, 1.09 and 1.18, respectively). In higher maximum monthly dose reduction velocity categories, associations were noted for higher event rates for overdose and mental health crisis.

Study limitations include unmeasured factors that may contribute to an increase in the risk of adverse events in the tapering population, an inability to assess tapering circumstances, and the lack of an accurate measure of race in the study population.

“These findings suggest that adverse events associated with tapering may be relatively common and support [Health and Human Services] recommendations for more gradual dose reductions when feasible and careful monitoring for withdrawal, substance use, and psychological distress,” the researchers concluded.

HealthDay News — Among individuals with cardiovascular disease, alcohol consumption is associated with all-cause and cardiovascular mortality and cardiovascular events in a J-shaped manner relative to nondrinkers, according to research published online July 27 in BMC Medicine.

Chengyi Ding, from University College London, and colleagues assessed alcohol consumption with respect to all-cause mortality, cardiovascular mortality, and subsequent cardiovascular events in an analysis of 14,386 patients with previous myocardial infarction, angina, or stroke from the U.K. Biobank Study (1,640 deaths; 2,950 subsequent events); 2,802 patients from the Health Survey for England (1,257 deaths); and three waves of the Scottish Health Survey. This was augmented with findings from 12 published studies, including data on 31,235 patients, 5,095 deaths, and 1,414 subsequent events. The combined sample included 48,423 patients.

The researchers found that alcohol consumption was associated with all assessed outcomes in a J-shaped manner relative to current nondrinkers; the risk reduction peaked at 7, 8, and 6 g/day for all-cause mortality, cardiovascular mortality, and cardiovascular events, respectively (relative risks, 0.79, 0.73, and 0.50, respectively) and remained significant up to 62, 50, and 15 g/day, respectively. Reductions in risk among light-to-moderate drinkers were attenuated in the few studies that excluded former drinkers for the nondrinking reference group.

“Our findings suggest that people with CVD may not need to stop drinking in order to prevent additional heart attacks, strokes, or angina, but that they may wish to consider lowering their weekly alcohol intake,” Ding said in a statement.

Among women with postpartum depression, zuranolone was found during a phase 3, double-blind trial to improve core symptoms of depression and was well tolerated. These findings were published in JAMA Psychiatry.

Women (N=153) with postpartum depression were recruited within 6 months of giving birth at 27 centers in the United States between 2017 and 2018. Participants were randomly assigned in a 1:1 ratio to receive zuranolone 30 mg (n=77) or placebo (n=76) for 2 weeks. Through day 45, patients were assessed with use of the 17-item Hamilton Rating Scale for Depression (HAMD-17).

The mean ages of patients in the zuranolone and placebo groups were 29.3±5.4 and 27.4±5.3 years old, respectively, and 58% and 54% were White. Body mass index (BMI) was 31.1±6 and 30.3±8 kg/m², 21% and 18% used antidepressants, and baseline HAMD-17 total scores were 28.4±2 and 28.8±2 points, respectively.

At day 15, the change from baseline in HAMD-17 scores was -17.8 points among the active treatment cohort and -13.6 points among the placebo group (P =.003). This significant difference was found to be sustained 4 weeks after treatment cessation on day 45 (least-squares mean difference, -4.1; 95% CI, -6.7 to -1.4; P =.003).

The response rate on day 15 was 72% among the zuranolone and 48% among the placebo cohorts (odds ratio [OR], 2.6; 95% CI, 1.3-5.2; P =.005), and remission rates were 45% and 23%, respectively (OR, 2.5; 95% CI, 1.2-5.2; P =.01).

Stratifying patients into subgroups of age, ethnicity, antidepressant use, BMI, onset of postpartum depression, or family history had no effect on the efficacy of zuranolone compared with placebo.

Adverse events were reported by 60% of the zuranolone and 52% of the placebo recipients. The most frequent adverse events reported by participants receiving active and control treatments were somnolence (15% vs 11%), headache (9% vs 12%), dizziness (8% vs 6%), upper respiratory tract infection (8% vs 1%), and diarrhea (6% vs 3%), respectively.

The longer-term efficacy of zuranolone for the treatment of postpartum depression remains unclear, but there is an ongoing open-label trial to address this question.

The study authors concluded that zuranolone rapidly decreased core symptoms of depression among women experiencing postpartum depression within 6 months of giving birth.
